Since October 2010, Crossroads Community SOURCE has been providing enhanced case management services to Medicaid SSI recipients in the Middle Georgia Area. Our Medical Director is Dr. Appavuchetty Soundappan of Perry, Georgia.
Crossroads Community SOURCE is committed to helping those who meet nursing home level of care but wish to remain at home. We help coordinate our members' healthcare needs through case management services. Along with assistance from a family member, neighbor, or friend and monthly contacts of our case managers, members in our program are able to enjoy the comforts of home as long as possible.
Our Mission is to provide members who have cognitive and/or functional limitations with quality care that enables them to live happier, healthier lives at home. Our services are distinguished by the responsiveness and expertise of our clinical case managers in recognizing each member has individual needs and interest. We embrace a positive, balanced approach to the evolving needs and changes within our industry to better accomplish the needs of those we serve.
The SOURCE program operates under the authority of the Georgia Elderly and Disabled 1915c Medicaid Waiver and provides Home and Community Based Services to frail elderly and physically disabled people who meet the Intermediate Nursing Home Level of Care.
Crossroads Community SOURCE links primary medical care and case management with approved long-term health services in a person's home or community. All SOURCE clients must be private pay or eligible for full Medicaid and meet nursing home level of care.
A comprehensive assessment determines eligibility and how much care a participant needs. Upon admission, an individual care path is designed based on the need for medical monitoring and assistance with functional tasks. Family members and other informal caregivers, as well as, staff from support agencies participate in care paths. In addition to core services, SOURCE support agencies offer personal support services, assisted living services, extended home health, home delivered meals, adult day healthcare, emergency response services, and 24-hour medical access to a case manager and primary care physician.
