Member Services
- Complete nursing home assessment to determine services needed and to develop an individualized plan of care, known as a Care Path, and monthly contact with case manager to make sure needs are being met.
- Communication between Case Manager, Primary Care Physician, and Service Provider to ensure coordination of all services.
- 24 hour access to Case Manager and Primary Care Physician
- Coordination of Personal Support Services such as assistance with bathing, dressing, meals, some household chores, and medication administration.
- Coordination of Nutritional Assistance and Home Delivered Meals
- Assistance with obtaining an Emergency Response System
- Coordination of Adult Day Health Services
- Coordination of Personal Care Home and Assisted Living Services
- Coordination with hospital Case Managers for discharge planning
- Coordination of Home Health Care and Skilled Services
- Coordination of transportation to and from medically related appointments
- Coordination of Caregiver Respite
Member Requirements
- Residents of these Counties: Atkinson, Bacon, Ben Hill, Berrien, Bibb, Bleckley, Brooks, Coffee, Colquitt, Cook, Crawford, Crisp, Dodge, Dooly, Dougherty, Houston, Irwin, Jeff Davis, Jones, Lanier, Laurens, Lee, Lowndes, Macon, Mitchell, Monroe, Peach, Pulaski, Sumter, Taylor, Telfair, Thomas, Tift, Turner, Twiggs, Ware, Wilcox, Wilkinson, or Worth
- Age 65 and older or under 65 and disabled who are SSI and Medicaid eligible
- May or may not have Medicare along with SSI
- Chronic medical conditions and/or disabilities that meet nursing home level of care criteria
- May be Private Pay
